This template outlines what should be included in the Quarterly Progress Report, which is due every 90 days after the agency signs and accepts the waiver terms and conditions during the developmental phase (i.e., before implementation begins), and what should be included in the Semiannual Progress Report, which is due every 180 days after implementation begins.
