We retrospectively reviewed and analyzed all hemodynamic\n\t\t\t\tdata in a large university hospital over a 5-year period to determine the\n\t\t\t\tfrequency and prognosis of the various causes of low SVR. Fifty-five patients\n\t\t\t\twith unexplained hypotension and a SVR less than\n\t\t\t\t800dynes\u00d7s/cm5were identified.\n        \n        \n          Main results:\n          Twenty-two patients (Groups 1 and 2) met the criteria for sepsis\n\t\t\t\tsyndrome. The mean SVR for this group was 445 \u00b1\n\t\t\t\t168 dynes\u00d7s/cm5 with an associated mortality of 50%. Group 3\n\t\t\t\tcontained 20 patients with possible sepsis. Thirteen patients (Group 4) were\n\t\t\t\tnonseptic. The mean SVR of this group was 435 \u00b1\n\t\t\t\t180 dynes \u00d7 s/cm5 with an associated mortality of 46%. Extremely\n\t\t\t\tlow SVR (below 450 dynes \u00d7 s/cm5) was associated with a\n\t\t\t\tsignificantly higher mortality regardless of the etiology.\n        \n        \n          Conclusions:\n          At least a quarter of patients with hypotension and a low SVR have\n\t\t\t\tnonseptic etiologies. The patients with nonseptic etiologies have a similar\n\t\t\t\tmortality to septic patients.
